INDICATIONS
It is a macrolide antibiotic medium spectrum bactericidal and effective action against a number of aerobic and anaerobic bacteria , Gram-positive and Gram-negative , so it is specific for the treatment of infections of the upper and lower respiratory tract . Clarithromycin is useful for the treatment of :

Upper respiratory tract infections (tonsillitis , pharyngitis , sinusitis , otitis ) .

Lower respiratory tract infections (bronchitis, pneumonia).

Infections of skin and soft tissue infections (folliculitis , cellulitis , erysipelas , impetigo ) .

Localized or disseminated infections , caused by Mycobacterium avium or M. intracellulare .

Contraindications
Clarithromycin is contraindicated in patients with known hypersensitivity to clarithromycin , erythromycin or any other macrolide antibiotics. Concomitant administration of clarithromycin and any of the following drugs is contraindicated : cisapride , pimozide , rifabutin and terfenadine .

DOSAGE AND ADMINISTRATION
Oral .

The recommended in infants , pediatric suspension of clarithromycin daily dose is 7.5 mg / kg / twice daily up to 500 mg every 12 hours for 5 to 10 days , depending on the pathogen involved and the severity of the infection .

Treatment for strep throat must be at least 10 days.

In children with renal impairment , creatinine clearance <30 ml / min , the dose should be reduced by half, to 250 mg every 24 hours or 250 mg every 12 hours in more severe infections , not more than 14 days .

In children with mycobacterial infections the recommended dose is 15-30 mcg / kg / day divided every 12 hours. Treatment should be continued as long as clinical benefit is demonstrated. The addition of other antimicrobial agents may be beneficial .
